Provide a statement of approximately 1,000 words describing the project and how the fellowship would aid it, including a statement of how much work has already been done on the project and how much would be completed during the fellowship period. 

In the same PDF, combine the statement above with a brief summary (300 words) contextualizing how the examples you are submitting through the "Documentation of Work" section below relate to the proposed project.

Provide samples or documentation of previous work, whether explicitly related to this project or not. These may be images, writing samples, or other content. There is a 5MB limit to uploads on this portal: if you'd like to submit files (video, sound or otherwise) that are larger than this, please email us for instructions.
